Patient Forms Packet Online
Our new patient
forms packet consists of all the information, registration
forms and the medical history questionnaire we have to get completed
before your appointment at Boland Eye Center. The packet is in PDF
format and you will need Adobe Reader on your computer to open it.
If you don't currently have Adobe Reader click on the Adobe button
below for your free download of the program.
Once you have
downloaded the packet you can printout the forms and fill in the required
information. Please bring these forms with you to your
appointment. If you don't have these forms with you, you will
have to fill them out at the office and it may delay your
appointment time. We suggest patients arrive 15 minutes
prior to the appointment time for insurance verification and
paperwork completion to expedite your visit with us. Thank you
